Important Waivers and Forms

COVID-19 Liability Waiver

Each family needs to complete this waiver to attend RSA or any RSA event in person.

Enrollment Liability Waiver

For parents wishing to apply for a waiver for students out-of-age for a class

Tuesday High School Drop-Off Program

This form needs to be completed by high school students (14+) taking classes on Tuesdays
